Job Description:

We're seeking a highly motivated, self-starter to provide integrated support to patients and pharmacists by providing a "hands-on" approach to total quality patient care. The Pharmacy Liaison will work closely with patients to deliver a full continuum of medication adherence support and encourages patients of specialty clinics to utilize pharmacy to fully benefit from its superior patient care services.

Responsibilities include:

* Providing "legendary" care to hospital patients by providing a full suite of custom medication management services
* Working seamlessly alongside hospital clinicians to serve patients both face-to-face and over the phone
* Educating prospective pharmacy patients on how hospital serves patients and improves their overall care
* Supporting outpatient retail and specialty prescription medication

Requirements include:

* 2+ years of experience as a Pharmacy Technician
* High School Diploma or GED required
* Strong interpersonal communication skills, ability to work independently and demonstrate good judgement
* Knowledge of transplant, hepatitis C, infectious disease (including HIV), and/or oncology a plus
